rheumatiod arthritis as a child, major surgery inlcudeing bone fragemtn removal and f;uid drainage, hysterectomy at age 30 for multiple ovarian cysts. major mva in 03/09 with bimallear fracture to right foot surgery included 2 plated and 10 screws, broke vertebrae 5 and 6, surgeries resulting in fusion of c5, c6, c7 with 1 plate and 6 screws,

fracture to left shoulder, still ecperiencing frequent pain, frayed rotator cuff. 5 broken ribs, lacreated liver, cut to head requiring 15 staples, still experiencing frequent pain and discomfort. frequent headaches since accident.
