I am 38 years old. I have frequent childhood illness. I had scarlet fever as well as frequent fevers from unknown causes. I moved around a lot during childhood and my whole life. I have a lot of Alzheimers and dementia as well as diabetes on my fathers side. I was diagnosed with gallbladder disease at age 17 and had to have it and my appendix out at 21. I was allergic to the codeine based medication. I almost die during surgeries because of the anesthesia. I have a heart murmur. Ihave sleep apnea and had a baby that died of SIDS. I started smoking at age 14 and I am a meth addict. I have 3 kids but my tubes are tied and burned. From the meth I suffered at different times from different psychoses and disorders. I have numerous diagnoses that are all different. I have the beginnings of a cataract. I have Graves Disease. I had radiation in 2002. I am on levothyroxine 125 mgs. per day. I get frequent sinus infections. I have constant joint pain. I have numbness on my back and pain
